Application Information
This scholarship program awards two scholarships up to $4,000 per year for four years, totaling a maximum
of $16,000 over four years for each student.

The criteria for the selection of these students is as follows:
- The student must be from one of the following high schools: Bridgeport, Liberty, Lincoln, Notre Dame, Robert C. Byrd and South Harrison.
- The student has achieved an overall GPA of at least 3.0 during his/her high school years.
- The student has participated in either sports, music, drama, community service, or other extracurricular activities.
- The student's parents/guardians have not collectively earned more than $40,000 per year. An additional $5,000 per year of earnings may be taken into consideration for each additional dependent. A copy of the student's FAFSA must be included. All information will be kept confidential.
- An interview with a small panel from the Hart Family Foundation will be conducted in May.
- The student may select any accredited 4-year college or university of his/her choice, or any 2-year community college (scholarship will be good for 2 years in this case).
- The up to $4,000 per year scholarship will be paid directly to the student's college or university to help cover the student's costs. Payments of up to $2,000 will be made each semester.
- The criteria were established for 2018 graduates.
The criteria to continue receiving the scholarship in college are as follows:
- The student must earn at least 12 credit hours per semester in college to remain eligible.
- The student must maintain a GPA of at least 2.5 for each semester for the first year and a GPA of at least 3.0 for each semester thereafter to remain eligible.
- The student is required to supply a copy of his/her transcript each semester (December and May) to remain eligible. These grades must be submitted to Dr. Donna Hage.

Along with your application, please include:
- A completed copy of your Free Application for Federal Student Aid (FAFSA).
- A copy of your high school transcript, including GPA and class rank.
- A copy of your ACT scores.
- A resume or list of achievements and school activities that demonstrate personal leadership characteristics.
- A list of community service activities/projects. Include names of organizations where you were involved, brief description of activity, dates of involvement, and the name and phone number/email address of a contact person.
- Submit up to three recommendation letters from individuals (not relatives) who are knowledgeable about you. These letters must be sealed and mailed directly from the individuals to:
